Click add to cart on this sample request and checkout as usual. During checkout, select Sample Request for shipping method to receive $15 flat rate shipping.
NOTE: Shipping rates will not be shown if more than 3 items are added in addition to this sample request. Remove sample request from cart if ordering more than 3 items.
Samples are sent separately to U.S destinations for a flat fee of $15 to cover shipping and handling cost. Samples sent separately to destinations outside the U.S. will also be charged $15, however, an additional charge will be added later to cover the cost of shipping.
